One possibility I can think of is antimicrobial resistance. Which means the bacterial is never cleared in the first place. But none are doctors here. Usually doctor give antibiotics and it is assumed to be better but the bacterial may not be totally clear. Just my guess why this kept coming back on and off. The other is cross infection.

Which is husband spread to wife, then wife spread to husband and husband spread to wife again. To solve this, both must see same doctor then make sure both are cleared. Is STd panel is negative, then it is not STD
